Professor Hübner, how many men suffer from incontinence problems after prostate surgery?
After radical prostatectomy there is a risk of incontinence problems. In the era of open surgery the percentage was much higher. With minimally invasive surgery methods and robotic surgery the risk is now much lower. Nevertheless, about 5 % of the patients need therapy.
What are the social implications of this problem?
The social implications are severe. Men who suffer from incontinence are considered old and sick, they withdraw from social life, they do not do any sports, sexuality is extremely problematic. These patients may be severely depressed.
Can every patient with an incontinence problem after prostatectomy be helped?

Incontinence problems can be solved in the vast majority of cases or at least be improved to a point where the patient comes back to social life.
What is the key to successful treatment?
A tailor-made therapy plan is essential for successful treatment – depending on the severity of the problem, age and other conditions. Treatment options should not be limited by the repertoire of the surgeon.
Which are the most common treatment options?
There are a number of treatment options – from simple slings to adjustable slings, adjustable balloons, sphincters with an open-close-mechanism. The decision which method will be used is based on the severity of the problem as well as on contraindications. Not every method is suitable for every patient. That is why it is so important to have all common methods available.
How long does the operation last, how long the hospital stay?
The operations are not very invasive and do not last long. A balloon implantation takes about 15 minutes, a sphincter implantation about 45 minutes. The hospital stay lasts maximum two days.
Can incontinence be treated after a longer time, even after several years?
Yes, incontinence after prostate surgery can be treated any time. If a patient is still incontinent six months after prostate surgery something should be done. However, it can also be treated after five or ten years after prostate surgery.
Does incontinence treatment interfere with ongoing cancer therapy?
No, it doesn’t. Even if a patient gets chemotherapy incontinence treatment is possible between cycles because it is so minimally invasive.
